What happens when you begin the Hospice referral process?

While the decision on when to consider hospice care may seem overwhelming, everyone at Goldencare Hospice & Palliative Care strives to make the hospice referral process as accommodating as possible, anticipating your needs whenever we can and addressing all of your concerns.

The hospice referral process usually starts with a phone call to our office at (855)655-0500. While hospice care requires a doctor’s order, anyone can make a referral—doctors, nurses, caregivers and the patient themselves often make the initial call. Additionally, family members, friends and neighbors all may make a referral or ask questions about when to consider or how to get hospice care for loved ones. After the initial call, a Goldencare Hospice & Palliative Care representative will meet with you or your loved one and family to fully explain our hospice philosophy and services. We will confirm medical eligibility and insurance coverage, as well as verify patient and family preferences for palliative/hospice care. If hospice care is determined to be the appropriate kind of medical care, paperwork will be completed and services begin—usually within 4 hours of referral.

Hospice Admission Process

When you have so many questions, we strive to make things easier. At Goldencare Hospice & Palliative Care, we understand how stressful this process can be. That’s why we are dedicated to making our hospice care admission process as smooth and hassle-free as possible.

Once we receive a referral, a registered nurse and social worker will visit you or your loved one and family in the home. We’ll conduct an evaluation, fully explain our services, answer questions, address concerns and apply the benefit of years of experience helping patients and families make a decision about hospice care. We also discuss with the treating doctor to verify that hospice care is the correct choice. Once primary care physician agrees with treatment, hospice care can begin almost immediately, generally in four hours or less.

We also offer an urgent admission option to those with critical care needs. A registered nurse, social worker and any other member of the Goldencare Hospice & Palliative Care team can be at your home within two hours of receiving a physician’s order.
